Work Environment

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians and create individualized treatment plans that align with their healthcare objectives.
• Administer a variety of therapeutic interventions at patients' homes, utilizing techniques that promote functional improvements in alignment with the physician's plan of care.
• Implement evidence-based practices while collaborating with interdisciplinary teams to maximize patient outcomes, demonstrating exceptional service and efficiency.
• Consult, educate, and guide patients and their caregivers on therapeutic exercises and lifestyle modifications to enhance their overall well-being.
• Document patient progress and outcomes meticulously, ensuring compliance with healthcare regulations and organizational objectives.

Qualifications

• Completion of a physical therapy program accredited by the APTA or CAPTE.
• Possession of a Bachelor's degree in Physical Therapy.
• Valid State License in Physical Therapy, and current Basic Life Support Certification from the American Heart Association.
• Excellent written and verbal communication skills, complemented by proficiency in word processing and standard computer applications.
• Strong ability to establish rapport with patients, ensuring their concerns are understood and addressed effectively.

Preferred Experience

• A minimum of 2 years of professional experience as a Physical Therapist, preferably in a home care or outpatient setting.
