Case Manager/Intake Specialist

Coalition for Responsible Community Development

Under the leadership and direction of the Ruth’s Place Manager, the Case Manager/Intake Specialist will provide services to homeless transition aged youth (18-24 years old). They will determine eligibility of all clients enrolling with the Coalition for Responsible Community Development for services, provide case management services, answer inquiries, and connect youths to housing and other resources. Job Type Full-time Essential Duties & Responsibilities Certify client documentation as prescribed by LASHA regulations, organize client files, and maintain case notes. Conduct orientations and information sessions for small and large groups. Coordinate presentations and enrollment of participants with mandatory partners and cross‑functional teams. Review enrollment packets for completeness and accuracy. Determine accurate participant enrollment into the correct programs. Conduct outreach and marketing to promote Ruth’s Place services and programs, including job fairs, community events, and partner agency events. Oversee the intake office, provide outreach, intake, and assessment of clients, and provide referral services. Attend meetings and trainings to improve skills and knowledge of best practices in Permanent Supportive Housing and client‑related services. Answer inbound calls in a timely and courteous manner. Be knowledgeable and familiar with the Homeless TAY population. Provide exceptional customer service to new and existing consumers. Enter client demographic information into HMIS and Salesforce database systems as needed. Maintain contact with partner agencies via email or telephone follow‑up. Coordinate in‑person assessments and admission with the Ruth’s Place team. Work occasional weekends and provide coverage for other positions when staff is short. Maintain department and program presentation for information sessions, community outreach, and inter‑departmental outreach. Develop and organize enrollment packets with current and updated forms mandated by LAHSA. Uphold CRCD’s Mission Statement and 5‑Year Strategic Plan. Participate in Care Coordination Meetings for SPA 6 and additional community meetings. Represent CRCD/Ruth’s Place at community resource events as needed. Work independently and with a multidisciplinary team in a fast‑paced environment. Link youths to resources such as education, employment, benefits, mental health, crisis bridge housing, and transitional housing. Formulate HSPs that identify youths’ needs and goals to reduce barriers to homelessness and secure housing, education, and employment. Enter weekly case notes in HMIS to document progress toward rapid transition to permanent housing. Complete monthly updates with youths to assess progress toward goals defined in the HSP. Assist youths with obtaining supporting documents and becoming document‑ready for housing. Comfortable working with TAY homeless or at‑risk populations. Demonstrate compassion for TAY, efficient time management, task prioritization, sound judgement, attention to detail, flexibility, and self‑drive. Requirements Knowledge of database management and quality control processes. Reliable, flexible team player who works well with minimal supervision and maintains personal boundaries. Ability to work on multiple projects simultaneously and meet deadlines. Knowledge of community resources. High flexibility and adaptability to changing priorities. Excellent organization and ability to use multiple databases simultaneously. Reliable transportation with valid driver’s license and car insurance as required by law. Bilingual in English/Spanish preferred but not required. Competence with computers, software programs, and mobile technology. Member of target population a plus.
