RN-Case Manager

The RN-Case Manager plays a critical role in coordinating and managing patient care to ensure optimal health outcomes. This position involves assessing patient needs, developing individualized care plans, and collaborating with multidisciplinary teams to facilitate effective treatment and resource utilization. The RN-Case Manager acts as a liaison between patients, families, healthcare providers, and insurance companies to advocate for appropriate care and support services. By monitoring patient progress and adjusting care plans as necessary, this role helps reduce hospital readmissions and promotes efficient use of healthcare resources. Ultimately, the RN-Case Manager ensures that patients receive comprehensive, timely, and cost-effective care throughout their healthcare journey.

Minimum Qualifications:

• Current and valid Registered Nurse (RN) license in the United States.
• Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
• Minimum of two years clinical nursing experience, preferably in case management or a related healthcare setting.
• Strong knowledge of healthcare systems, insurance processes, and regulatory requirements.
• Excellent communication, organizational, and critical thinking skills.

Preferred Qualifications:

• Certification in Case Management (CCM) or Accredited Case Manager (ACM) credential.
• Experience with electronic health records (EHR) and case management software.
• Background in chronic disease management or utilization review.
• Master's degree in Nursing, Healthcare Administration, or related field.
• Familiarity with community resources and social services.

Responsibilities:

• Conduct comprehensive assessments of patients' medical, psychosocial, and environmental needs to develop personalized care plans.
• Coordinate with physicians, nurses, social workers, and other healthcare professionals to implement and monitor care plans.
• Facilitate communication between patients, families, and healthcare providers to ensure understanding and adherence to treatment plans.
• Manage transitions of care, including hospital discharge planning and referrals to community resources or specialists.
• Review and analyze patient records and insurance information to authorize services and ensure compliance with regulatory requirements.
• Provide education and support to patients and families regarding disease management, treatment options, and available resources.
• Document all case management activities accurately and timely in accordance with organizational policies and legal standards.

The RN-Case Manager utilizes clinical expertise and critical thinking skills daily to assess patient conditions and develop effective care plans. Strong communication skills are essential for coordinating with multidisciplinary teams and educating patients and families about treatment options. Organizational skills enable the management of multiple cases simultaneously while ensuring compliance with documentation and regulatory standards. Proficiency with electronic health records and case management software supports efficient tracking and reporting of patient progress. Additionally, negotiation and advocacy skills are employed to secure necessary services and resources for patients, ensuring comprehensive and patient-centered care.
