Case Manager, RN (Portland, OR / Field-Based)

Our mission spans coast to coast, supporting patients across the U.S. Overview The Case Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and PCA education, and quality assurance to ensure safe, effective, and person-centered care. Responsibilities Conduct initial in-person clinical assessments and coordinate service activation, ensuring that all required forms and standard procedures are followed. Develop, implement, and manage the patient’s activity plan(s). Make revisions and document changes as needed. Complete all client documentation, including written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise, and legible manner. Actively coordinate the care of the client through communication and written documentation to the physician and other disciplines involved in the client’s care and community resources. Provide ongoing coaching to drive effective utilization of established best practices, encourage PCA education, and connect families to appropriate resources. Review the caregiver notes daily and appropriately address changes in the consumer’s condition or environment. Ensure continuous compliance with all regulatory standards as well as company policies and procedures. Perform other job‑related duties as may be required or directed by the supervisor. Education and Experience Diploma from an accredited nursing program. New graduates welcome! Additional Skills / Experience (Plus) Home care case management Care coordination experience Bilingual Qualifications Valid and active Registered Nursing License issued by the Oregon Board of Nursing. Ability to articulate cases through effective summaries. Ability and flexibility to travel (usually by car) within assigned geographic area. Valid driver’s license and car insurance required. Proficiency in common software and web‑based applications (e.g., Case Management Systems, MS Office) with the ability to teach others. A passion for providing a high level of customer service, including the ability to show empathy, active listening, and patience.
