Collaborate with multidisciplinary teams to ensure optimal patient outcomes through proactive care management and evidence-based practices. Engage in a mission-driven organization dedicated to transforming healthcare for seniors through innovative care models. Work onsite Monday-Friday with no weekend or after-hours commitments, ensuring a balanced professional schedule. Manage a patient panel of approximately 400 individuals, focusing on personalized care and measurable health improvements. Participate in leadership development opportunities to influence healthcare delivery and organizational culture.Primary Care Physician Key Responsibilities & Duties Provide comprehensive care for patients with acute and chronic conditions, ensuring adherence to evidence-based practices. Lead a care team to deliver coordinated, patient-focused healthcare services, including education and health promotion. Collaborate with specialists and hospitalists to manage patient care across various settings, including hospitals and long-term care facilities. Maintain accurate and concise documentation using advanced healthcare technologies to support care delivery. Participate in quality improvement initiatives and risk management programs to enhance patient outcomes. Engage in recruiting efforts to expand patient panels and support organizational growth. Supervise and collaborate with Advanced Practice Practitioners, ensuring compliance and accountability in patient care. Attend clinical and operational meetings to align with organizational goals and standards.Primary Care Physician Job Requirements Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in Internal Medicine, Family Medicine, or Geriatrics. Active State Medical License and DEA number for controlled substances. Board certification in relevant specialty with ongoing maintenance of certification. Basic Life Support (BLS) certification within 90 days of employment. Proficiency in Microsoft Office Suite and healthcare documentation systems. Strong commitment to patient-centered care and outcomes-focused healthcare delivery. Ability to travel locally for patient care and nationally for training or recruitment. Fluency in English, with additional language skills preferred based on location needs.
