Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

Loudoun Free Clinic is seeking a compassionate and experienced Advanced Practice Provider (APP) to provide high-quality primary care to uninsured adults. The APP will work collaboratively with our clinical team to deliver patient-centered care while supporting the Clinic’s mission of improving access to healthcare. Location is in person at our facility in downtown Leesburg. Key Responsibilities Provide comprehensive primary care for patients with acute and chronic health conditions. Promote preventive care and support patients in achieving their health goals. Educate patients about diagnoses, treatment plans, medications, and healthy lifestyle choices. Prescribe medications in accordance with Virginia regulations and practice agreements. Refer patients to specialists and community resources when appropriate. Collaborate with providers, staff, volunteers, and community partners to coordinate patient care. Maintain accurate and timely documentation in Athena electronic health records (EHR). Participate in quality improvement initiatives. Support the mission, values, and policies of Loudoun Free Clinic. Minimum Qualifications Master’s degree as a Family or Adult Nurse Practitioner from an accredited program with an active Virginia license, or Physician Assistant certification with an active Virginia license. Prescriptive authority in the Commonwealth of Virginia. At least two years of experience in a primary care setting. Current Basic Life Support (BLS) certification. #J-18808-Ljbffr
