Responsibilities:

• Deliver person-centered nursing care in alignment with Mayo Clinic standards.
• Monitor and manage residents' health needs across multiple care settings.
• Collaborate with interdisciplinary teams to ensure resident safety and well-being.
• Accurately document skilled interventions to reflect clinical judgement and resident needs.

Qualifications:

• Licensed Practical Nurse (LPN) : Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
• Registered Nurse (RN) : Graduate of an Accreditation Commission for Education in Nursing (ACEN) accredited associate's degree or diploma program.
• Active Minnesota Licensed Practical Nurse or Registered Nurse License
• Basic Cardiac Life Support Certification for Health Care Providers

Additional Qualifications:

• One year of work experience in Long Term Care or Home Health or two years of acute care nursing is preferred.
• Ability to adapt to a dynamic environment and prioritize resident needs.
• Ability to work independently.
• Strong commitment to teamwork, communication, and resident-centered care.
• Excellent communication and human relation skills to interact with Health Service and Charter House staff.
• Strong problem-solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process for groups of residents and employees.
• Ability to adapt to unpredictable situations and stress.
• Works flexible hours, which may include days, evenings, nights, holidays and weekends.

Compensation Detail:

Education, experience and tenure may be considered along with internal equity when job offers are extended.
