Job Summary The Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during a patient’s hospital stay and coordinates planning with other disciplines utilizing a patient/customer driven approach. Responsibilities Perform comprehensive nursing assessments and reassessments. Conduct age‑appropriate admission or transfer assessments, obtaining input from families or guardians when appropriate. Document findings accurately and completely. Assess post‑operative and post‑invasive procedure patients. Assess and document education and discharge needs of patients and families on admission and throughout hospitalization. Reassess patients and document pertinent observations according to the patient plan of care, changes in condition, status or diagnosis, response to care, procedures, and standards of care. Establish, coordinate, and evaluate a plan of care based on assessment data, patient diagnosis, lab results, tests, physician orders, protocols, standards of care, and other relevant information. Identify short‑term and long‑term goals based on patient care needs. Formulate nursing interventions to achieve desired patient outcomes. Incorporate disease‑specific evidence‑based practice into the nursing care plan and documentation. Provide and document nursing interventions based on assessed patient needs, the plan of care, and changes in patient status. Collaborate with health team members to coordinate the daily plan of care for assigned patients. Provide, coordinate, and communicate patient care, including accurate handoff communication reports. Administer and document medications accurately according to policies and procedures. Monitor, maintain, and document accurate IV fluids and parenteral nutrition according to policies and procedures. Complete referrals as indicated by assessment data. Request consultation for specialized needs, equipment, or patient/family information. Provide patient/family education and discharge planning according to documentation guidelines and protocols. Clarify physician orders as warranted; acknowledge and implement orders accurately and in a timely manner. Assist physicians with procedures or treatments as requested or delegate to a care partner as appropriate. Document “Readback” for all telephone or verbal orders; take verbally ordered instructions only in emergency situations. Recognize changes in patient condition and take appropriate nursing actions. Involve family or guardian in care and decision‑making when appropriate. Identify risks to patients and take appropriate actions. Incorporate infection control practices into daily care. Document or communicate changes in nursing care or patient condition. Perform ongoing evaluation of care effectiveness based on assessment data, nursing interventions, patient responses to medications, treatments, and procedures. Evaluate the effectiveness of patient and family education. Modify the plan of care as indicated. Recognize significant changes in clinical parameters and report immediately to the physician and others as needed. Identify problems, gather relevant data, suggest solutions, communicate with appropriate lines of authority, and work toward resolution. Report variations from care or treatment following reporting policies and procedures. Provide specialized care to patients at high risk for injury, including restraint care, pain management, and abuse assessment. Initiate, evaluate alternatives to restraints, apply restraints per approved procedures, monitor patient response, provide specified care, and document use thoroughly. Assess for pain, incorporate cultural and spiritual beliefs into pain management, implement techniques, and involve patients and families in pain management planning. Recognize and respond to signs of abuse or neglect, reporting to the appropriate authorities and taking necessary actions to support patient safety. Adhere to all quality and performance standards, policies, procedures, and protocols for clinical and technical aspects of care. Engage in learning experiences to increase professional competence. Demonstrate appropriate technical and cognitive skills for the area of practice. Maintain currency in hospital and unit information, policies, and procedures. Attend staff meetings and participate in committees, shared governance, and work teams. Review nursing publications and keep updated on policies and procedures. Adapt positively to changing work demands and support colleagues. Provide instruction to new employees and maintain a collaborative team approach. Serve as a resource to co‑workers, patients, families, and medical staff. Assume and delegate care responsibilities appropriately and communicate precise plans of care. Supervise and teach students or new staff nurses. Perform job in accordance with safety and infection control policies, practicing standard precautions and disposing of hazardous waste per guidelines. Maintain a safe, clean, comfortable, and therapeutic environment for patients and staff. Follow hand hygiene procedures, use safe patient handling techniques, keep noise levels low, adhere to dress code, and report risk management concerns. Complete all annual mandatory requirements, including maintaining current licenses and certifications, BLS, HI, TB screening, influenza vaccination, ethics training, safety training, information security, abuse and neglect courses, bloodborne pathogens training, coursework, competency evaluations, and other relevant training. Provide service excellence by demonstrating care and courtesy, offering prompt, respectful, and courteous service, meeting the physical, spiritual, psychological, and emotional needs of patients and families, collaborating with the health care team, and ensuring timely response to patient requests. Share knowledge with other departments, use resources wisely, recommend improvements to increase efficiency, and ensure customer satisfaction through established hospital principles. Incorporate performance improvement and evidence‑based practice into professional practice, assist in developing and implementing unit goals, and participate in data collection, performance improvement, and nursing research activities. Provide age‑appropriate care, demonstrating knowledge of normal growth and development, assessing age‑specific data, involving family in decision‑making, and delivering services tailored to the age groups served by the department. Use resources efficiently, prioritize responsibilities, and complete work on time, documenting chargeable supplies and equipment used. Requirements Licensed Registered Nurse in the state of Missouri. Basic Life Support HealthCare Provider (BLS HCP) – American Heart Association (AHA). Additional certifications as required by each state of work location (see below). Completion of annual mandatory training including BLS, HI, TB screening, influenza vaccination, ethics training, safety training, information security, abuse & neglect course, bloodborne pathogens training, and competency evaluations. Job Specifications Work Shift: PRN / Per Diem (United States of America). Department: Clinical Outreach (code 8251010037). Scheduled Weekly Hours: 0. Working Conditions: May be exposed to the risk of blood borne diseases, unpleasant environments, accidents, injuries, illnesses, unpredictable situations, emergency and crisis situations, and irregular hours. Physical Requirements: Occasional prolonged standing and walking, lifting, positioning, pushing, transferring patients, reaching, stooping, bending, kneeling, and crouching.
