Make a meaningful impact delivering compassionate care to patients in their homes. This full-time, 13-week contract role is ideal for a Registered Nurse with expertise in the home health setting, eager to provide dedicated, field-based care using OASIS assessments and Home Care Home Base (HCHB).

Schedule & Expectations:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (typically 35 patient visits per day)
• Field-based: Daily travel to patient homes within an assigned territory (mileage reimbursement provided)

Essential Qualifications:

• Active RN License in good standing within the state of practice
• Minimum of 1 year of recent Home Health experience (preferably with field visits)
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Demonstrated competency with OASIS assessment tool
• Reliable transportation, valid driver's license, and auto insurance

Core Responsibilities:

• Conduct comprehensive OASIS assessments; develop and implement personalized care plans
• Deliver skilled nursing interventions, including wound care, medication administration, and IV therapy
• Monitor patient conditions, document progress, and communicate changes to the interdisciplinary team
• Educate patients and families regarding diagnoses, treatments, medications, and preventative practices
• Maintain accurate, timely medical records in HCHB
• Collaborate actively with therapists, social workers, and home health aides for coordinated care
• Attend required case conferences, staff meetings, and professional development activities
• Uphold all regulatory and agency standards for patient care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
