Home Care Field Nurse (RN) - per diem

We are seeking a part-time Registered Nurse (RN) to join our dedicated home care team. Our private-duty agency provides personal care (non-medical) services to the elderly in Morris and Essex counties. The field nurse will perform in-home client assessments, create individualized care plans, and orient and monitor aides providing services to our clients. Benefits: · Flexible, part-time schedule · Competitive pay · Mileage reimbursement · Paid time off · Referral bonuses Job duties: * Assess new clients, develop plans of care, and orient Certified Homemaker-Home Health Aides to cases. * Conduct client reassessments every 60 days, or more frequently if the client’s condition warrants. * Assess Aide competency and observe their performance every 3 months. * Maintain documentation per New Jersey Board of Nursing/NJ Division of Consumer Affairs regulations and ACHC accreditation standards. * Be available for off-hour phone consults (very rare). Requirements: * Current RN license in good standing in NJ. * Valid NJ driver’s license and reliable vehicle to get to clients’ homes. * Minimum two years RN experience, including one year of home health care experience preferred. * Excellent clinical, assessment, organizational, and communication skills. * Ease with computers. * Some schedule flexibility.
