LPN - Patchogue, New York
Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Swan Lake Nursing & Rehabilitation in Patchogue, New York. As an LPN, you will play a vital role in delivering high-quality patient care, working closely with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
Provide a full range of healthcare services, including assessment and treatment of common acute and chronic health problems, in accordance with current standards of nursing practice
Supervise, coordinate, and evaluate the health care team and services on the unit
Collaborate with other healthcare professionals to develop and implement individualized care plans
Provide guidance and support to subordinate personnel
Maintain accurate and detailed records of patient care
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
Current licensure as a Licensed Practical Nurse in the State of New York
Graduate of an approved NLN accredited nursing program or State Approved Vocational Program
Ability to provide leadership and supervision to subordinate personnel
Genuine interest in gerontology and an understanding of the special needs of the geriatric elder
Successful completion of pre-employment health screening and background check
CPR certification
Additional Benefits:
$38 - $40 HOURLY salary range
