This role combines hands on patient care, education, and collaboration with physicians and care teams to support safe, coordinated, and high quality outcomes outside the hospital. Core Responsibilities Maintain and apply advanced knowledge of respiratory equipment, ventilators, HME products, and related payer and regulatory guidelines Perform clinical assessments and reassessments, including review of ventilator patient records, compliance data, and care plans; communicate findings and recommendations to physicians and referral sources Participate in discharge planning and execution for highly technical and ventilator dependent patients, ensuring smooth transitions to the home setting Demonstrate and instruct patients, caregivers, and healthcare professionals on ventilator units, supplies, and proper equipment use Provide clinical follow up with physicians and referral sources regarding patient status and tolerance of therapy Conduct clinical #J-18808-Ljbffr AdaptHealth, LLC
