RN Care Manager

Role Description The RN Care Manager is a full-time, on-site role based in Atlanta, GA, responsible for coordinating and managing patient care across the primary care continuum. This role includes assessing patient needs, developing individualized care plans, and providing education on disease management, medications, and lifestyle changes. The RN Care Manager collaborates with physicians, advanced practice providers, and other care team members to ensure safe transitions of care, reduce hospital readmissions, and support chronic condition management. Daily tasks involve monitoring clinical progress, performing follow-up outreach, documenting in the electronic health record, and advocating for patients to access appropriate community and specialty resources. The RN Care Manager also participates in quality improvement initiatives and supports evidence-based care standards. Qualifications Strong nursing foundation with demonstrated skills in Nursing and Patient Care. Experience in Nursing Management, including care coordination and team collaboration. Knowledge of Medicine and Critical Care Medicine principles relevant to primary and chronic care management. Active Registered Nurse (RN) license in the state of Georgia. Prior experience in primary care, care management, or a similar clinical setting preferred. Ability to communicate clearly with patients, families, and interdisciplinary teams. Proficiency with electronic health records and basic clinical documentation standards.
