Job Title: Contingent Clinical Care LVN/LPN PT - MC Address: Exton, PA Job Description Administer oral, injectable, intravenous, and topical medications. Evaluate complex patient conditions, monitor pain levels, and document treatment responses. Apply advanced knowledge of vital sign monitoring to identify and record abnormalities. Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources. Conduct medication reviews to identify potential side effects and drug interactions. Accurately document patient medical history using electronic health records. Critical Information This is an in-person, patient-facing role. Training hours to be determined; regular shifts between 7-10 hours in clinic operating hours. Education/Licenses Needed A full, active and unrestricted license as a Licensed Practical or Vocational Nurse. Benefits Benefits are available to full-time employees after 90 days of employment.
