Hours: 25 patient-facing hours weekly / 40 hours per week
Location: New Hampshire (Territory-based)

Position Overview

We are seeking an experienced Licensed Medical Social Worker (LMSW) with recent hospice experience to provide compassionate support and care coordination for geriatric patients in a home hospice setting. The ideal candidate will be comfortable traveling throughout the assigned territory, maintaining timely documentation, and collaborating with the interdisciplinary care team.

Schedule & Caseload

• Monday – Friday
• 40 hours per week
• Approximately 5–7 patient visits per day (please confirm during interview)
• No weekends
• No on-call requirements

Required Qualifications

• Active New Hampshire LMSW license in hand
• Minimum 1 year of recent Hospice Social Work experience
• Experience with HCHB (Homecare Homebase) EMR required
• BLS Certification required
• Strong documentation and communication skills
• Must be flexible with assigned territory coverage

Patient Population

• Primarily Geriatric Hospice Patients

Territory

• Coverage area may extend up to a 60-mile radius from the assigned office/hub
• Traveler must be willing to float within the designated territory as needed
