Location: St. Cabrini Nursing Home 115 Broadway Dobbs Ferry, NY 10522 Patient Population: Adult and Geriatric EMR: Crown Web Dress Code: Dialyze Direct will provide black scrub tops. Scrub bottoms: Personal scrubs of any color No RTO allowed since this is a shorter assignment. Non-billable orientation: 8hrs Holiday and OT Multipliers: 1.25% Call, Call-Back and Charge: Not applicable for this need

Submission Requirements 1 year or more current dialysis experience - Required Copy of current, valid driver license - Required Geriatric patient experience - Required NXStage Dialysis Machine experience Required - Please note experience on submission CNN or CDN Certification - PREFERRED Crown Web EMR experience - Preferred BSN - Preferred Profile Requirements: Work History Skills Checklist (pertinent to position), current to 1 year 1 supervisory signed reference (from within the past two years of employment) License/Certifications - Must be uploaded with profile Certification Requirements BLS (AHA) State License Requirements New York Additional License Requirements Must be in hand upon submission
