Job Description

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own schedule to fit your lifestyle
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments in patient homes throughout Adams County, including locations such as Quincy, Camp Point, and Payson
• Complete assessments focused on annual health or post-discharge visits, with each evaluation typically lasting about 45 minutes
• Review and document prior and current medical history, existing conditions, and medication use
• Provide patient education on health conditions and proper medication management
• Identify gaps in care and make recommendations to address and resolve these issues
• Note: This position involves evaluation and education onlythere are no treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner License in Illinois
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum of 6 months of NP experience is preferred
