The Care Team Hospice is seeking an experienced and relationship-focused Hospice Clinical Liaison (RN/LPN) to help strengthen our partnerships with hospitals, skilled nursing facilities, senior living communities, physicians, and other healthcare providers. This is a unique opportunity for a clinician who enjoys being out in the community, building relationships, educating others, and connecting patients with the right care at the right time. As a Hospice Clinical Liaison, you'll serve as the bridge between our clinical and sales teams, bringing your healthcare expertise directly to our referral partners. You'll maintain a consistent presence within assigned facilities, provide hospice education and clinical consultation, identify patients who may benefit from hospice services, and help ensure a smooth transition from referral through admission. QualificationsKey Responsibilities Build and maintain strong relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other healthcare referral partners. Serve as a clinical hospice resource by providing education, consultation, and guidance on hospice eligibility, disease progression, symptom management, and the Medicare Hospice Benefit. Maintain a consistent presence within assigned facilities, identify patients who may benefit from hospice services, and support timely referral and admission coordination. Partner closely with Community Care Liaisons, clinical leadership, admissions, and operations to remove barriers, strengthen accounts, and improve referral-to-admission conversion. Conduct clinical screenings and hospice eligibility assessments within RN scope of practice while ensuring appropriate communication and documentation throughout the referral process. Qualifications Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required. Two or more years of hospice, palliative care, home health, skilled nursing, geriatric care, or related healthcare experience preferred. Strong knowledge of hospice care, eligibility criteria, and the Medicare Hospice Benefit. Excellent communication, presentation, relationship-building, and customer service skills. Prior healthcare liaison, admissions, business development, or sales support experience preferred. Ability to work independently, manage an assigned territory, and maintain a strong presence with referral partners. Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory required. Travel: Ability to perform regular travel within assigned geography and extended geography for training and meetings. Transportation: Must have reliable transportation.
