Licensed Practical Nurse (LPN)

The Citadel of Skokie is seeking a Licensed Practical Nurse (LPN) to join their team. As an LPN, you will provide care to residents, performing assessments, implementing and evaluating plans of care, and ensuring the highest quality of nursing service. Licensed Practical Nurse (LPN) Duties and Responsibilities Provides licensed care to assigned residents as ordered by the physician and in accordance with facility, federal, state, and local standards, guidelines, and regulations. Conducts resident assessments/observations and intervenes, reviewing care plans to ensure appropriate care. Records vital signs, administers catheter care, IV therapy within license limits, and performs respiratory care such as oxygen and nebulizer treatments. Documents nursing notes thoroughly, reflecting care provided and resident responses. Maintains a safe environment for residents, promptly addressing potential safety risks. Monitors and directs non‑licensed ancillary services to meet professional standards and maintain high levels of resident care. Communicates and reports information to residents, families, physicians, guardians, governmental agencies, and facility personnel per policy and state/federal guidelines. Ensures all aspects of resident care plans are implemented and maintained, recommending changes to the interdisciplinary team as needed. Enforces infection control standards and promptly corrects non‑compliance. Adheres to company policies and procedures regarding HIPAA compliance. Performs all other duties as assigned. Licensed Practical (LPN) Nurse Qualifications and Skills Excellent written and verbal communication skills in English. Ability to multi‑task and remain calm under stress. Strong analytical and problem‑solving skills. Familiarity with infection control standards, policies, and procedures. Flexibility to work a varied schedule and occasional overnight travel. Positive, effective, and appropriate interaction with residents, families, community members, volunteers, and facility staff. Licensed Practical Nurse (LPN) Education and Experience Nursing degree or diploma from an accredited college or university. Current license to practice as a Licensed Practical Nurse in the state. Preferred: 1 year of experience; Long‑Term care experience preferred. Physical Requirements Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, and ability to distinguish smells and temperatures; all may be performed with or without mechanical aids. Limited exposure to environmental hazards; must understand and follow company safety standards and protocols.
