Professional Nurse

With a commitment to patient- and family-centered care, the professional nurse uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. Assists in leading the daily operations of a specific unit. Acts as a resource, lead, and clinical role model for the patient care team. Advances and oversees patient care standards of the unit for the designated shift. Also facilitates and coordinates patient care delivery and flow through the continuum of care. Supports nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program.

Job responsibilities include:

• Interacting with co-workers, visitors, and other staff consistent with the values of Jefferson. Recognizing the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.
• Communicating with and supporting the interprofessional teams and services, patient, family and designated care partners. Serving as a clinical resource regarding policies and procedures, patient standards, performance improvement, and risk management issues.
• Collaborating with the medical staff to facilitate consistency of interdisciplinary plans of care to ensure quality patient care. Assessing patient needs and making care recommendations based on the patient's physical, psychosocial and cultural needs, age and functional status. Facilitating timely patient throughput optimization for all phases of care including admissions, transfers, and discharge optimization of patients on the designated unit. May participate in plan of care rounds depending on service and unit protocols.
• Providing timely Smart Square schedules and on call schedules if applicable. Following up appropriately and effectively resolves customer and physician issues and complaints.
• Monitoring staff schedules to ensure adequate staffing to meet patient care needs and maintain quality patient care. May authorize overtime or scheduling changes to fill shifts as necessary. Completes payroll in collaboration with Nurse Manager. Provides volumes & adjustments to reflect variances in staffing needs within Smart Square.
• Collaborating with the Nurse Manager to evaluate, maintain and improve patient care team member performance. Assists in implementing changes in clinical practice and evaluates the quality and effectiveness of nursing practice to ensure compliance with organization, department and unit specific goals.
• Supporting nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program. Providing patient care support when necessary.

Qualifications include:

• Education: Specialized Diploma Graduate of an accredited school of nursing required.
• Education: Bachelor's Degree BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred.
• Experience: 3 years as a Registered Nurse in an acute care setting.
• Knowledge, Skills and Abilities: Effective Communication, Patient-and Family-Centered Practice, Cultural Awareness, Care Coordination, Patient Safety, Medication Administration and Prescription, Discharge Planning, Assessment and Diagnosis, Health Information Documentation.
• Licenses and Certifications: BLS - Basic Life Support AHA - American Heart Association, RN - Licensed Registered Nurse_PA - State of Pennsylvania or RN - Licensed Registered Nurse_NJ - State of New Jersey.
• Additional Qualifications: Active RN license required in state of practice.
