Registered NurseThe Registered Nurse requires substantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological, physical, and social sciences in the application of the nursing process in the respective departments in which assigned. The RN must collaborate with physicians and multidisciplinary team members; provide physical and psychological support to patients, friends, and families; supervising assigned team members and students.ResponsibilitiesCompletes patient assessments, formulates plan of care, oversees implementation of plan of care, evaluate and adjust plan of care utilizing critical thinking skillsThe performance of other nursing acts which are authorized or limited by the board, and which are consistent either with American Nursing's Association Standards of Practice or with Standards of Practice established by nationally accepted organizations of registered nurses.Care, counsel health teaching and promotion, utilizes Teach Back methodology as affirmation of comprehensionThe administration of medication, intravenous therapy and treatment as prescribed by a LIPSupervision, delegation and follow-through to other personnel in the performance of activities relating to nursing care.Mentors and precepts students and orient new staff as assignedReports up chain of command Clinical Nurse Manager/Nurse Manager/House Nurse Coordinator pertinent informationParticipates on various teams, committees or performance improvement initiativesEnsures a safe and secure environment for patients, coworkers and selfParticipates in Discharge Planning and Transition of Care (able to define pt needs)Serves as a patient advocate and escalates patient related concerns timelyTimely documents pertinent findings in medical recordAssist patients in performing activities of daily living and functions as an active team memberAssumes responsibility for personnel professional developmentUtilizes effective communication skills and active listening, exhibits compassionate and empathetic attitudePractices Hands Off communication utilizing SBAR; Performs Bedside Shift reporting utilizing SBAR (unless contradicted)Functions as a positive influence, role model and adheres to professional code of conductRespects dignity and confidentiality of all matters relating to patientsRefer to Special Addendum and Staff Nurse Position DescriptionPerforms other job duties as assigned.QualificationsEducation• Associate's Degree required.• Bachelor's Degree preferred.Licenses & Certifications• Registered Nurse required.• BLS certification required upon hire.• De-Escalation Training required within six (6) months of hire.• Driver's License required.Required Skills, Knowledge & Abilities• Responsible for preparation of and interpretation of client plans of care and systematic assessments/reassessments.• Responsible for performing nursing treatments, procedures and total client care in accordance with established agency and nursing care standards.• Responsible for communicating pertinent client assessment/reassessment findings to the physician.• Responsible for initiating referrals to other healthcare organizations and community services based on identified client needs and physician orders.• Able to travel to client residences in inclement weather and provide nursing care in varying environmental situations.• Ability to work independently while coordinating care with physicians, patients, families, and interdisciplinary healthcare teams.• Strong clinical assessment, critical thinking, and decision-making skills.• Ability to communicate effectively and document patient care accurately.• Ability to prioritize workload and manage multiple patient needs in a home health environment.
