CNA- Long Term Care- Full Time

Plymouth Harbor

The Certified Nursing Assistant (CNA) is responsible for providing routine nursing care in accordance with established Plymouth Harbor policies and procedures as directed by the Charge Nurse, Director of Nursing Services, and/or the Administrator of Health Services, in assuring the highest possible degree of quality nursing care.

Schedule: Mid shift, 3pm-11pm

DUTIES/RESPONSIBILITIES
  1. Make walking rounds at beginning and end of shift.
  2. Perform all assigned duties in accordance with established policies and procedures and nursing care procedures.
  3. Assist residents with dental and mouth care.
  4. Assist in preparing residents for meals.
  5. Ensure that resident's food is accessible, self-help devices are available, and food is appropriately arranged for visually impaired residents.
  6. Assist with tray set-up and feeding of patients. Remove trays in a timely manner.
  7. Perform after-meal personal care as required.
  8. Record resident's food intake as directed.
  9. Assist residents with personal hygiene and grooming needs in accordance with resident's plan of care.
  10. Assist residents with Activities of Daily Living, including toileting, dressing, and bathing, in accordance with resident's plan of care.
  11. Assist licensed nurses, as directed, in collection of specimens, i.e., stools, urine, sputum, etc.
  12. Assist in the transfer of residents via mechanical lift from bed to chair, chair to toilet, etc.
  13. Give back care as instructed.
  14. Observe and report any insult to skin.
  15. Record and/or report intake and output as directed.
  16. Keep incontinent residents as clean and dry as possible at all times. Change bed linens and clothing as often as necessary.
  17. Check and report bowel movements and character of stools as directed.
  18. Make beds, clean bedside tables, tidy immediate resident area, and clean bathrooms after used for nursing procedures. Remove all linens and disposable items from resident area upon completion of care.
  19. Assist resident in preparing for schedule appointments/planned activities, e.g., x-rays, lab work, physician visits, or religious services.
  20. Transport residents to and from designated activities by appropriate means.
  21. Assist in or perform in activities and nursing procedures, as instructed by Charge Nurse or Nursing Supervisor.
  22. Observe principles of safety and body mechanics at all times while at Plymouth Harbor.
  23. Report all resident complaints to the Charge Nurse.
  24. Check residents at designated intervals, according to current need and condition, and report findings to the Charge Nurse.
  25. Report any unusual or questionable behavior of a resident to the Charge Nurse.
  26. Assist other staff members when indicated or requested.
  27. Immediately report any abuse to the Charge Nurse.
  28. Maintain a comfortable environment for the residents. Note changes in room temperature, ventilation, or lighting, and adjust to the resident's satisfaction. Report actions to the Charge Nurse.
  29. Record resident information in the electronic medical record according to Smith Care Center policy and as directed by the Charge Nurse.
  30. Ensure that the Resident's Bill of Rights is followed at all times.
  31. Report all accidents/incidents to the Charge Nurse.
  32. Report all hazardous conditions and equipment to the Charge Nurse.
  33. Receive assignments and check resident schedule at beginning of shift and plan accordingly.
  34. Follow Universal Precautions and Infection Control procedures; adhere to all isolation measures when identified.
  35. Maintain current licensure through State of Florida Department of Health.
  36. Attend and participate in workshops, seminars, and mandatory in-services as approved or required.
  37. Maintain work area, equipment, and supplies in a clean, sanitary, and organized manner.
  38. Work safely; complying at all times with Plymouth Harbor safety standards.
  39. Call 555 in the event of an emergency.
  40. Discharge properly other responsibilities and duties that may be directed by Charge Nurse, Shift Supervisor, Director of Health Services, or the Vice President of Health Services.

SUPERVISORY RESPONSIBILITY
  • This position has no supervisory responsibilities.
QUALIFICATIONS
  • Graduate of an accredited technical school for Certified Nursing Assistants.
  • Minimum one year in the field of Long-Term Care preferred.
  • Must have good organizational skills.
  • Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
  • Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual's competence.
  • Must have good communication skills, speaking, writing, and listening.
  • Must be fluent in English.
  • Must have good computer skills with the ability to learn and use proprietary software as required.
CERTIFICATES/LICENSES/REGISTRATIONS
  • Must have valid certification as a Nursing Assistant in the State of Florida.
  • Must have a current CPR (Cardiopulmonary Resuscitation) certification.
  • Must have a valid Florida Driver's license.

  • Maintain high personal standards for performance and encourage others to do the same.
  • Must be able to get along with others and work as a team player.
  • Maintain confidentiality in all Plymouth Harbor, resident, and team member matters.
  • Use good judgement and make independent decisions when circumstances warrant such action.
  • Work harmoniously with all persons residing in, employed by, or associated with, the organization.
  • Be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people.
  • Wear Plymouth Harbor team member i.d. badge at all times when on duty.
  • Abide by the dress and grooming guidelines established for the department and possess good personal hygiene habits.
PHYSICAL REQUIREMENTS
  • Must possess fully functioning sense of sight/hearing or use prosthetics that will enable these senses to function adequately in order to do the job.
  • Must be able to sit, stand, walk, and move throughout the workday (lifting, carrying, bending, squatting, reaching, kneeling, pushing, pulling, twisting) using good body mechanics.
  • Must be able to lift/push up to 100 pounds (with assistance if lifting).
  • Must be able to cope with the physical and emotional demands of the position.
  • Important Background Screening Information:
    As required by Florida law (House Bill 531), candidates must complete a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse as a condition of employment.
