Caregiver Support Specialist (Merit Award Trust Fund) Full-Time / Non-Exempt / Full Benefits Pay Range = $20.00 - $22.00 per hour (Depending on Experience) Our Mission To provide a comprehensive and coordinated system of services designed to promote the independence and dignity of older persons and their families in Muskegon, Oceana, and Ottawa counties a mission compelling us to focus on older persons in greatest need and to advocate for all. GENERAL DESCRIPTION The Caregiver Support Specialist (CSS) will work with caregivers to ensure their emotional and physical needs are met with the goal of extending their ability to provide care. Through the use of evidence-based programs for caregivers, peer support group facilitation, and outreach the Caregiver Support Specialist will build rapport with caregivers and link them to resources within the community. ESSENTIAL DUTIES AND RESPONSIBILITIES Classes and Groups Provide Powerful Tools for Caregivers a minimum of twice per year. Provide training and/or education tailored to the individual/or group needs of caregivers. Develop support groups in areas that are currently underserved. Support groups may be led by lay leaders once established with support. Support groups will have a written schedule with topics for discussion and learning. Outreach Create and implement outreach to employers, hospitals, independent and assisted living, nursing homes, medical providers, senior centers, congregate meal sites, and churches. Coordinate with the Communications Department to develop and distribute flyers and other materials in the community to support caregivers. The caregiver support specialist will work with support coordinators to collect the caregiver needs assessment on the person-centered service plan. The CSS will contact the caregiver to offer support and services through one-on-one intervention, support groups, online resources, and classes. Individual support The Caregiver Support Specialist will conduct in-home assessments and develop plans with the caregiver to ensure the caregiver's needs are met. GUIDE (Guiding an Improved Dementia Experience) Model Provide support and care as part of an interdisciplinary GUIDE team. Provide educational classes and respite services for GUIDE beneficiaries. QUALIFICATIONS REQUIREMENTS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Acquire and maintain Caregiving Facilitator Certification (GUIDE required). Must have reliable transportation and provide current proof of automobile insurance. Must possess a cell phone. Must have a test for tuberculosis upon hire and yearly symptom reviews thereafter. Must pass all background checks.
