How You'll Make an Impact in This Role

Clinical Excellence & Advisory: Assess, review, and optimize complex medication orders to proactively resolve drug-related issues, recommending formulary alternatives and serving as a subject matter expert for interprofessional care teams and clinical leadership.

Specialized Dispensing & Safety: Oversee the precise compounding, dosing, and dispensing of medications, ensuring rigorous safety standards across specialized patient populations.

Interdisciplinary Collaboration: Direct-consult with physicians, nurses, and care managers to streamline treatment plans and provide expert drug information to healthcare staff.

Patient Advocacy & Education: Empower patients through direct medication counseling, helping them safely navigate and understand their therapy regimens.

Operational Agility: Adapt to dynamic clinical needs by providing flexible coverage across pharmacy units when required.

What Minimum Requirements You'll Need

Licensure/Registration/Licensure:

• Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.

Education:

• Bachelor's or advanced degree in Pharmacy required.
• Doctor of Pharmacy
• Minimum: 2 years of ambulatory care/chronic disease management experience.
• Preferred: 3-5 years of ambulatory care/chronic disease management experience and BCACP.
