Registered Nurse

Job Title: Registered Nurse (RN) – Hospice Location: Cordova TN, 38018 (Coverage: Shelby, Fayette, Tipton Counties) Schedule: Monday–Friday, 8:00 AM – 4:30 PM Position Overview seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality hospice care to patients in their homes. This role focuses on case management, symptom control, and interdisciplinary collaboration to ensure comfort and quality of life for hospice patients. Key Responsibilities Manage a caseload of hospice patients, ensuring comprehensive care planning and coordination Perform symptom management and provide clinical interventions to enhance patient comfort Conduct patient visits (6 visits per day) and complete thorough assessments Prepare for and actively participate in Interdisciplinary Team (IDT) meetings Collaborate with physicians, social workers, chaplains, and caregivers to deliver holistic care Provide patient and family education on disease progression, symptom control, and end-of-life care Maintain accurate and timely documentation using Homecare Homebase (HCHB/PointCare) Travel within assigned counties to provide in-home hospice services Requirements Active Registered Nurse (RN) license Prior hospice experience Experience with Homecare Homebase (HCHB/PointCare) Strong clinical assessment and case management skills Ability to work independently and manage patient schedules effectively Valid driver’s license and reliable transportation Preferred Qualifications Flexible and adaptable, with a patient-first mindset Strong communication and interpersonal skills Ability to handle emotionally sensitive situations with compassion and professionalism #J-18808-Ljbffr
