Duties

• Conduct comprehensive assessments of patients’ physical abilities, medical history, and current condition to develop tailored treatment plans.
• Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, and therapeutic exercises to promote recovery.
• Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care and discharge planning.
• Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
• Educate patients and their families on treatment goals, home exercise programs, and community resources to support ongoing rehabilitation.
• Monitor patient progress regularly, adjusting treatment plans as needed to optimize outcomes for conditions like orthopedics, neurology, pediatrics, geriatrics, and post-surgical recovery.
• Assist with discharge planning by evaluating patient needs for continued care at home or in community care settings.

Requirements

• Valid state licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role.
• Proven experience in outpatient clinics, inpatient hospital settings, or home health environments with knowledge of acute care standards.
• Strong understanding of anatomy, physiology, medical terminology, and clinical documentation standards.
• Experience working with geriatric patients, pediatrics or neurology populations is preferred.
• Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for efficient documentation.
• Ability to communicate effectively with clients and healthcare teams while demonstrating empathy and professionalism.
• Knowledge of treatment modalities including orthopedics, trigger point therapy, and community-based rehabilitation practices.
