Location: Douglas, GA | Coverage Area: Coffee and Bacon Counties

Schedule: Full Time | Hybrid | Travel required

Pay: $43,888

Position Summary

The Support Coordinator assists individuals with intellectual and developmental disabilities in accessing, coordinating, and monitoring community-based services. This role focuses on care coordination, service planning, and ongoing support to help individuals achieve personal goals and maintain quality outcomes.


Key Responsibilities

• Coordinate and manage individualized service plans (ISP)

• Conduct assessments and gather clinical/social information

• Facilitate ISP meetings and ensure annual updates

• Connect individuals to community resources and services

• Assist with Medicaid eligibility and recertification

• Maintain monthly face-to-face contact per guidelines

• Monitor service delivery and outcomes

• Document services and maintain compliance with DBHDD and HIPAA

• Respond to participant needs, concerns, and crises

• Collaborate with families, providers, and interdisciplinary teams

Qualifications

• Bachelor's degree in human services field (required)

• 1+ year experience in case management or developmental disabilities services

• Master's degree may substitute for experience

• Valid driver's license and ability to travel locally

• Must reside within assigned coverage area (preferred)

Skills

• Strong communication and relationship-building

• Problem-solving and critical thinking

• Knowledge of Medicaid/waiver programs

• Microsoft Office and documentation skills

Work Environment

This is a mobile, field-based position requiring travel throughout the assigned counties. Work includes home visits, community settings, and occasional remote/administrative tasks. Flexibility for evenings or weekends may be required.
