As a Behavioral Health Case Manager at Noble, you will:

• Deliver Compassionate Care : Provide patient-centered case management and resource navigation to support and improve overall well-being.
• Be Part of an Integrated Care Model : Deliver evidence-based, quality care with the support of a collaborative team, including Medical, Pharmacy, and Dental providers.
• Serve as a Case Management Expert : Work closely with an interdisciplinary team while offering your expertise to address diverse needs.
• Expand Access to Care : Help break barriers to care by focusing on the needs of underserved communities.
• Innovate and Collaborate : Thrive in an environment that values innovation, continuously addressing the intersection of health and behavioral health needs.
• Shape the Future : Mentor and collaborate with students training at Noble, contributing to the growth of the next generation of care providers.

What You'll Do:

• Provide case management and follow-up care to support patients in meeting health and treatment goals.

• Collaborate with primary care and behavioral health providers to coordinate care, treatment plans, and patient education related to mental health, SUD, and psychosocial needs.

• Serve as a liaison across the integrated care team to improve communication and coordination of services.

• Assess Social Determinants of Health (SDoH) and connect patients to appropriate internal and community resources.

• Advocate for patients to help navigate systems and secure needed services.

• Support patient re-engagement and care coordination activities.

• Conduct secondary suicide risk assessments, develop safety plans, and assist with crisis de-escalation using trauma-informed approaches.

• Facilitate integrated care clinical consult team meetings.

What We're Looking For:

• Master’s degree in Social Work, or a related behavioral health field from an accredited program, preferred; Bachelor's degree in social work, with experience in case management or social services required.

• Licensure Requirements:
  Current Wisconsin licensure as a Certified Social Worker (CSW) or Advanced Practice Social Worker (APSW) , or eligibility to obtain licensure within six (6) months of hire .
  License must be active and in good standing with the State of Wisconsin .
