Schedule: Day Shift | Monday Friday | 8:00 AM 5:00 PM

About the Role

We are seeking an experienced Physical Therapist (PT) Home Health for a 12-week contract opportunity in Missoula, Montana. The Physical Therapist will provide skilled rehabilitation services in patients' homes throughout the assigned service area, including evaluations, treatment, care planning, patient and family education, and coordination with interdisciplinary healthcare teams.

The role requires travel between patient homes, point-of-care documentation, OASIS knowledge, productivity management, and supervision of Physical Therapist Assistants when applicable.

Responsibilities

• Evaluate patients in the home setting and develop individualized physical therapy plans of care based on functional needs, physician orders, and rehabilitation goals.

• Provide skilled physical therapy interventions, reassess patient progress, and modify treatment frequency, duration, and techniques based on clinical response.

• Complete OASIS assessments, point-of-care documentation, admissions, reassessments, and discharge documentation according to home health requirements.

• Coordinate care with physicians and interdisciplinary healthcare professionals while providing patient and family education and participating in care conferences.

• Supervise Physical Therapist Assistants as required, maintain productivity standards, and manage daily schedules, mileage, and travel routes efficiently.

Requirements

• Active Montana Physical Therapist (PT) License Required.

• Minimum 1 Year of Physical Therapy Experience Required; 2 Years Preferred.

• Degree in Physical Therapy From an Accredited Physical Therapy Program Required.

• Valid Driver's License, Reliable Transportation, and Appropriate Auto Insurance Required.

• Home Health Experience, OASIS Knowledge, and Homecare Homebase (HCHB) Experience Preferred.

The Physical Therapist will provide compassionate, evidence-based home health rehabilitation while supporting patient safety, functional independence, and improved quality of life.
