Care Manager II | Assisted Outpatient Treatment (132)

Job Purpose:
- The Care Manager II at LIFESTREAM BEHAVIORAL CENTER is responsible for providing comprehensive care management services to clients with complex behavioral health needs. This role involves coordinating care, facilitating communication among healthcare providers, and ensuring that clients receive the appropriate services to improve their overall well-being and quality of life.
- Develop and implement individualized care plans for clients, ensuring that all aspects of their behavioral health needs are addressed.
- Coordinate with healthcare providers, social services, and community resources to arrange for necessary services and support for clients.
- Monitor and evaluate client progress, adjusting care plans as needed to achieve optimal outcomes.
- Facilitate communication among clients, families, and healthcare providers to ensure a cohesive approach to care.
- Advocate for clients' needs and rights, ensuring they have access to the services and support necessary for their well-being.
- Maintain accurate and up-to-date documentation of client interactions, care plans, and progress reports.
- Participate in multidisciplinary team meetings to discuss client progress and collaborate on care strategies.
- Provide education and support to clients and their families to help them understand and manage their behavioral health conditions.
- Stay informed about the latest developments in behavioral health care and integrate evidence-based practices into care management strategies.
- Ensure compliance with all relevant regulations, policies, and procedures in the delivery of care management services. Required Education:
- Bachelor's degree in Social Work, Psychology, Nursing, or a related field. Required Experience:
- Minimum of 2 years of experience in case management or care coordination within a behavioral health setting.
- Experience working with diverse populations and understanding of cultural competence in healthcare.
- Proven track record of developing and implementing individualized care plans. Required Skills and Abilities:
- Strong knowledge of behavioral health principles and practices.
- Excellent communication and interpersonal skills to effectively interact with clients, families, and healthcare professionals.
- Ability to assess client needs and develop appropriate care plans.
- Proficient in using electronic health records and other healthcare management systems.
- Strong organizational skills and ability to manage multiple cases simultaneously.
- Ability to work independently as well as part of a multidisciplinary team.
