Nurse Practitioner - San Jose

Vivo HealthStaff is recruiting for a Nurse Practitioner in the San Francisco Bay Area. This is a permanent position with salary and benefits.

We are seeking a Nurse Practitioner to join a well-established non-profit organization providing healthcare services to the geriatric population throughout the Bay Area. The ideal candidate has experience working with older adult patients and is fluent in either Spanish, Cantonese, Mandarin, or Vietnamese.

Position Summary:
The Nurse Practitioner will support the delivery of comprehensive care by conducting health assessments, managing chronic conditions, and coordinating care for patients across outpatient and community-based settings. This role involves collaboration with a multidisciplinary care team to ensure high-quality, patient-centered services.

Schedule:

• Monday through Friday, 40 hours per week
• No nights, weekends, or call required

Key Responsibilities:

• Perform physical exams and health assessments for geriatric patients
• Collaborate with the care team to develop and implement patient treatment plans
• Manage minor illnesses and stable chronic conditions within NP scope of practice
• Evaluate acute symptoms and coordinate appropriate referrals
• Conduct home and nursing home visits as needed
• Provide clinical support and guidance to registered nurses
• Participate in health education, prevention, and wellness programs
• Attend regular team meetings and complete all required documentation

Qualifications:

• Active and unrestricted Nurse Practitioner and Registered Nurse license in California
• Minimum of one (1) year of experience as an RN
• At least one (1) year of experience working with the elderly population
• Supervisory or leadership experience is a plus
• Language skills required: Fluency in Spanish, Cantonese, Mandarin, or Vietnamese
• Ability to work effectively in a multidisciplinary team-based care environment
