Key Responsibilities:

• Conduct comprehensive in-home health risk assessments for Medicare Advantage members
• Review and evaluate individuals past and current medical conditions, history, and medication use
• Offer education and information regarding diagnosed health conditions and prescribed medications
• Detect potential gaps in care and provide actionable recommendations for improved patient outcomes
• No responsibility for treatment plan creation or medication prescribing

Qualifications:

• Active Nurse Practitioner (NP) license in New Mexico
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited program
• At least 6 months of current Nurse Practitioner experience (preferred)
