Registered Nurse- Long-Term Care

divh2Registered Nurse Opportunity/h2pCVP is seeking Registered Nurses to join our team in providing our Nations Veterans with long-term care services at the Cincinnati VA Medical Center-Fort Thomas. The RN will utilize their broad based clinical knowledge to effectively assess, implement health promotion and prevention interventions, manage acute and chronic illness states, assist patient/residents in the attainment of optimal levels of functioning rehabilitation, and evaluate patient/resident response to actual or potential health problems./ppWe have full time, part-time and PRN contract opportunities available./ph3Responsibilities/h3ulliUtilize the nursing process in managing, developing, implementing, and evaluating each residents individual plan of care with the interdisciplinary team to attain specific clinical outcomes/liliProvide a full range of nursing care to residents with a variety of physical and/or behavioral problems/liliDemonstrate leadership in delivering and improving holistic care through collaborative strategies with others/liliMaintain patient/resident confidentiality and a safe patient/resident care environment/liliAssess and act on significant changes in the patients/residents health condition/liliCollect and document pertinent data into the electronic patient/resident medical record using designated templates/liliComplete nursing assessments within required time frames and in accordance with policy and procedure/liliMeet the health care needs of patients/residents by assessing and assisting them to meet expressed needs/liliPrioritize and complete tasks within the assigned shift/liliUpdate patient/resident care plans and notify assigned physician of any changes in condition/liliPractice the VA core values of Integrity, Commitment, Advocacy, Respect, and Excellence (ICARE)/liliDocument patient progress and chart the patients response to medications and treatments/liliPerform wound care including dressing changes (sterile and non-sterile) and obtaining wound cultures as necessary/liliProvide care for tracheostomies, central lines, PICC lines, feeding tubes, and catheters as needed/liliPerform fall risk assessments and skin assessments (Braden Scale) to identify and prevent injuries/liliComplete required documentation within established timeframes/liliParticipate in quality improvement activities/li/ulh3Qualifications/h3ulliAbility to pass a government background investigation (US Citizenship required)/liliBachelors Degree in Nursing or a Masters Degree in Nursing required/liliCurrent, full, active, and unrestricted RN license in any State, Territory, or Commonwealth of the United States or District of Columbia/liliBasic Life Support (BLS) certification from the American Heart Association/lili2+ years providing Geriatric, Community Living or Long Term Care/liliTechnical proficiency including strong computer skills and ability to learn VA electronic medical record systems/li/ulpLocation: Cincinnati VA Medical Center-Fort Thomas, 1000 South Fort Thomas Ave, Fort Thomas, KY/ppAbout CVP: CVP is an award-winning healthcare and next-gen technology and consulting services firm solving critical problems for healthcare, national security, and public sector clients.
