Key Qualifications:

• Active RN license
• Home health experience strongly preferred
• Strong communication and collaboration skills

Core Responsibilities:

• Collaborate in the development, implementation, and revision of the case management program
• Act as a liaison among patients, families, physicians, and multidisciplinary teams to optimize care outcomes
• Lead coordination of patient care, ensuring timely and appropriate interventions
• Monitor adherence to interdisciplinary care plans and covered services, ensuring continuity throughout the patient's clinical course
• Consult with physicians regarding care utilization, treatment plans, and discharge processes
• Focus on supporting high-needs populations-including those with chronic pain and substance use disorders
• Coordinate transfers to tertiary centers, facilitating seamless transitions and effective information sharing
• Collect data for regulatory and accreditation compliance
• Conduct reviews on frequent emergency department patients to enhance care quality
