As a **Hospice Aide.,** you’ll contribute to our success in the following ways:* Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.* Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.* Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.* Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.* Document medication regimens and maintain accurate, up-to-date medication profiles.* Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.* Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team. **What is Required?*** High School Diploma or GED equivalent required* 1 year of experience as a Hospice Aide or Nursing Assistant is required* State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment* Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.* Experience in a home health environment is *preferred* **What's in it for you?*** Comprehensive insurance plans for medical, dental, and vision benefits including pet insurance* 401(K) with employer match* Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost* Paid time off, paid holidays, family, and pet bereavement* Fleet vehicle program (restrictions apply) or mileage reimbursement.\\* Benefit eligibility can vary by position and shift status.***Equal Employment Opportunity**: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
