Patient Care Coordinator

Facilitates the coordination of care for patients through the continuum based on their diagnosis or procedure which may include but is not limited to ensuring follow up with physician offices, scheduling of necessary diagnostic procedures providing education and emotional support throughout the path of care or designated timeframe. Triages incoming referrals for acuity to assist with patient throughput. Educates patients about their treatments and assists with overcoming barriers to healthcare.

Type: Part Time (48 hours per 2 week pay period, with benefits)

Typical hours for this position: Monday, Wednesday, and Thursday 8:00am-4:30pm

Education:

- Associate's Degree in Nursing- required

-Bachelor's Degree of Science in Nursing- preferred

License/Certification:

- Must hold an active RN license- required

- BLS- required

Experience:

- Minimum of three (3) years cardiology nursing experience- required

- Proficiency at triaging complex cardiology patients and understand general cardiology, interventional cardiology, as well as electrophysiology- required
