Description:
This position functions under the supervision of the Unit Director and the leadership of the Senior Nurse or designated Charge Nurse.
The Registered Professional Staff Nurse is responsible for providing individualized quality patient care by assessing patient needs through data collection.
Designs, implements, and continuously evaluates the plan of care.
Works in collaboration with all members of the treatment team.
Performs other related duties as assigned or requested.
May be assigned to any inpatient unit to meet patient care needs.
On admission, completes Nursing Admission Assessment: identifies strengths, weaknesses, and problems.
Develops Individualized Treatment Plans considering the patient's age, disability, ethnicity, culture, and religion.
Administers medication and treatment accurately according to policies and procedures.
Assesses the effect of medication on the patient.
Attends multidisciplinary treatment planning conferences for assigned patients.
Works collaboratively with members of the treatment team to achieve identified goals.
Assures patient care is provided in accordance with the Master Treatment Plan.
Understands and implements hospital-wide regulatory agencies and standard nursing policies, procedures, regulations, and standards.
Operates within professional scope of practice.
Effectively collaborates with the treatment team and positively supports patients' plans for continuing care.
Assesses learning needs, readiness, and barriers to learning for patients and families.
Documents appropriately.
Provides age-appropriate learning opportunities, both individual and group, and documents teaching and outcomes accordingly.
Provides educational materials that are specifically appropriate for patients and families, considering age, educational level, culture, and disability.
Provides input to the Unit Director regarding unit-based performance improvement measures based on concerns, high risk, and high-volume issues.
Participates in the unit's performance improvement activities by accurately collecting data focused on established indicators, processes, and outcomes.
Maintains a safe and orderly work and patient care environment.
Immediately reports all environmental and safety hazards as per policies and procedures.
Handles and disposes of infectious and hazardous materials according to policies and procedures.
Reviews all assigned patient charts for new orders at the beginning of each shift.
Accurately transcribes, reviews, and co-signs medication orders within procedural time limits.
Immediately documents adverse reactions and unusual incidents using appropriate reports and follows up as required by policies and procedures.
Uses approved hospital abbreviations for charting at all times.
Documents patient information within medical records accurately and within procedural time limits: assessments, progress notes, responses to care provided, treatment plans, updates, and discharge summaries.
Participates in the orientation of new and transfer personnel and students as assigned to the unit.
Identifies one patient problem, strength, and intervention within 24 hours of admission.
Attends treatment planning conferences when scheduled and makes additions or revisions to plans as discussed at meetings.
Reviews the Master Treatment Plan weekly and makes revisions as warranted by the patient's condition.
Plans and conducts nursing psycho-educational groups as per unit schedule.
Meets all requirements for annual IV recertification if applicable.
Models standards of behavior.
Demonstrates computer literacy and competency for all required systems.
Demonstrates positive communication techniques in working with other members of the treatment team to enhance patient care.
Must be able to demonstrate the knowledge and skills necessary to provide care to adolescent, adult, and older adult patients.
Demonstrates knowledge of key principles of growth and development across the lifespan.
Demonstrates cultural competence relative to each patient's status.
Must be able to interpret the appropriate information needed to identify patient needs relative to age, culture, ethnicity, disability, sexual orientation, and gender identity when applicable.
Understands patient orientation and admission processes.
Completes assessments for abuse, neglect, and domestic violence.
Conducts pain assessments.
Completes necessary documentation as required.
Knows different emergency codes and how to respond to them.
Knows CPR and how to use an AED.
Provides care in the least restrictive manner.
Understands and utilizes various emergency techniques.
Ability to effectively communicate verbally and in writing with adults and adolescents.
Completes incident reporting per organizational policy.
Understands and maintains all different unit-specific protocols, rules, and procedures.
Demonstrates safe patient handling techniques.
Maintains a therapeutic milieu and sets therapeutic limits effectively.
Demonstrates effective one-on-one interactions with patients.
Understands suicide prevention and assessment protocols.
Provides care in the least restrictive manner.
