Home-Based Behavioral Health Case Manager

Optum Home & Community Care, part of UnitedHealth Group, is expanding to deliver integrated care across physical, mental, and social needs. As a member of our Care at Home team, you will help patients navigate the health system and connect them with key supports. This role emphasizes fast-paced, measurable results within a diverse population.
