Dzilth-Na-O-Dith-Hle - Health Center

The Dzilth-Na-O-Dith-Hle Health Center is part of the Shiprock Service Unit located in the Easternmost part of the Navajo Nation. Inpatient care needs are met by referral to the Northern Navajo Medical Center in Shiprock, New Mexico, which is over 60 miles Northwest of the Health Center. The city of Farmington, New Mexico is less than an hour's drive from the Health Center, lying between the facility and Shiprock, New Mexico. The Health Center provides ambulatory care 40 hours per week (closed weeknights and weekends) to approximately 7,000 nearby residents. The Dzilth Health Center is staffed by three physicians and one physician's assistant. In addition to family medicine staff, services are provided which include laboratory, x-ray, pharmacy, mental health, public health nursing, dental, and optometry. Some staff lives in Indian Health Service provided housing and others reside in private homes in Farmington, New Mexico. The facility, while relatively new (10 years) is scheduled for an expansion due to begin in the fall of 1999.

Requirements (Duties Include But Are Not Limited To)

• The Contractor shall provide outpatient Internal Medicine physician services in the delivery of patient care to the Indian Health Service.
• The Contractor shall perform in accordance with the competency standards listed in the Shiprock Service Unit Medical Staff By-Laws.
• The Contractor shall provide professional medical services or direct patient care services under the terms of this contract, appropriate and timely medical services in accordance with the standards of care established by recognized medical care organizations and in accordance with the policies and procedures of Service Unit's Medical Staff Bylaws and Rules & Regulations at Shiprock hospital.
• The contractor shall have access to the internet to use the Electronic Health Record. All documentation on patients shall be completed prior to the contractor's shift being over. Delinquent charts shall be completed prior to the contractor's completion of his/her contract at Dzilth-na-o-dith-hle Health Center. If the delinquent chart is not completed the contract company shall assume the responsibility of returning the contractor to Dzilth-na-o-dith-hle Health Center to complete any outstanding delinquent chart.
• The contractor shall provide professional Family Medicine physician services indirect patient care services as follows.
• The contractor shall provide service in the DZHC Outpatient Department in the delivery of patient care. Service shall be inspected, reviewed, and monitored by the Clinical Director of the DZHC or her designee.
• The contractor shall take care of infants, children, adults, and geriatric patients with acute or chronic medical illnesses.
• The contractor shall not perform any surgeries except minor office procedures, if able.
• The contractor shall obtain a consultation with other members of the DZHC Outpatient Department staff on matters of patient care when questions arise.
• The contractor shall review clinical records and adhere to records documentation procedures to ensure that records are properly maintained.
• The contractor shall be subject to technical direction and/or guidance of the designated Government official.
• The contractor's provision of Family Medicine Care shall be monitored by the DZHC Clinical Director.

Qualifications

• Nurse Practise or Physician's assistant candidate must have completed appropriate education and training and be appropriately Licensed to meet Shiprock service Unit credentialing and privileging requirements.
• The Contractor shall provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to the personal health and safety of others.
• Current certification in Basic Life Support (BLS), Advanced cardiac life support(ACLS), and Pediatric life support (PALS).
