In this role, you'll work closely with physicians, therapists, and the interdisciplinary care team to assess wounds, develop treatment plans, prevent skin breakdown, and improve resident outcomes. Monday–Friday schedule (if applicable) Supportive leadership team Competitive pay DailyPay available Medical, Dental & Vision Insurance 401(k) Employee Recognition Programs Continuing Education Opportunities Stable, growing organization What You'll Do As our Treatment Nurse / Wound Care Nurse , you will: Complete comprehensive skin and wound assessments Develop and implement individualized wound treatment plans Perform wound care treatments and dressing changes Monitor wound healing and modify treatments as appropriate Lead weekly wound rounds with the interdisciplinary team Coordinate physician communication and obtain treatment orders Maintain accurate wound documentation and measurements Monitor residents for pressure injuries and skin integrity issues Educate nursing staff on wound prevention and skin management Ensure compliance with CMS, state, and infection prevention guidelines Coordinate wound care supplies and treatment needs Participate in Quality Assurance and Performance Improvement (QAPI) We’re Looking For Current RN or LPN license in the state Experience in skilled nursing, rehabilitation, long-term care, or wound care preferred Strong assessment and documentation skills Knowledge of pressure injury prevention and wound healing Current CPR/BLS Certification Excellent communication and teamwork skills Passion for improving resident outcomes Ideal Background Experience in any of the following is a plus: Wound Care Treatment Nurse Skilled Nursing Facility (SNF) Rehabilitation Long-Term Care (LTC) Subacute Care Post-Acute Care Geriatric Nursing Charge Nurse Infection Prevention Why You’ll Love Working Here At Willowcreek Wellness & Rehabilitation, we believe great nursing starts with great people.
