Social Worker: Elder Care & Benefits Advocate

Hollybank Trustees Ltd in Brentwood, NY, seeks a Social Worker to provide mission-aligned psychosocial services to Sisters across multiple living communities. The role coordinates benefits, long-term planning, MLTC, and transitions, collaborating with HCPs, nurses, and leadership to ensure appropriate care.
The position requires a CSWE-accredited MSW and NY LMSW/LCSW license, with strong assessment, advocacy, and coordination skills. On-site travel to three sites is expected.
