ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations./ppThe Care Manager plays a pivotal role in coordinating and managing comprehensive care plans for individuals requiring long-term support within the healthcare and social assistance sector. This position focuses on assessing members needs, developing personalized discharge plans, and ensuring seamless transitions between care settings to promote optimal health outcomes. The Care Manager collaborates closely with multidisciplinary teams, including healthcare providers, social workers, and community resources, to facilitate access to Medicaid and other support services. By advocating for members and monitoring their progress, the Care Manager ensures that care delivery aligns with both clinical guidelines and individual preferences. Ultimately, this role aims to enhance the quality of life for members by providing compassionate, patient-centered case management and support throughout their care journey./ppMinimum Qualifications:/pulliBachelors degree in Social Work, Human Services, Nursing, or a related field./liliExperience in case management within healthcare or social services settings./liliKnowledge of Medicaid policies and procedures./liliRelevant experience may substitute for the educational requirement on a year-for-year basis./li/ulpPreferred Qualifications:/pulliMasters degree in Social Work (MSW) or related discipline./liliCertification in Case Management (CCM) or equivalent./liliExperience working with diverse populations in community-based settings./liliFamiliarity with electronic health records (EHR) and care coordination software./liliTraining in trauma-informed care or behavioral health interventions./li/ulpResponsibilities:/pulliConduct comprehensive assessments of members physical, mental, and social needs to develop individualized care and discharge plans./liliCoordinate with healthcare providers, social service agencies, and community resources to facilitate access to Medicaid and other benefits./liliMonitor members progress and adjust care plans as necessary to address changing needs and ensure continuity of care./liliProvide education and support to members and their families regarding available services, treatment options, and long-term care planning./liliMaintain detailed documentation and case records in compliance with regulatory standards and organizational policies./li/ul/div
