RN Infusion Care Transition Specialist

Option Care

Option Care Health is seeking a Clinical Transition Specialist to educate patients and referral sources about at-home and alternate-site infusion services. You will assess patient suitability, coordinate with hospital teams, and support timely discharge with a strong focus on relationship-building and sales outcomes. The role requires RN or Pharmacist licensure with 2+ years in healthcare, plus strong communication and PC skills. Travel to partnerships and facilities is expected. #J-18808-Ljbffr Option Care
