Day Shift | Monday – Friday | 8:00 AM – 4:30 PM

A healthcare organization in Northern California is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment in Clearlake, CA. This role focuses on providing in-home therapy services designed to restore mobility, improve function, and support patients in maintaining independence within their home environment.

This is an excellent opportunity for a therapist who enjoys working autonomously while collaborating with interdisciplinary care teams.

Position Details

• Schedule: Monday – Friday, 8:00 AM – 4:30 PM

• Shift: Day shift (8 hours)

• Contract Length: 13 weeks

• Setting: Home Health

• Mileage Reimbursement: Paid at the federal government rate

• Weekend Opportunities: Optional shifts may be available

Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans

• Deliver therapy interventions to restore and improve functional mobility and independence

• Utilize appropriate clinical techniques and treatment protocols

• Establish care plans and discharge plans while coordinating with interdisciplinary teams

• Document patient care and progress accurately and within required timeframes

• Educate patients and caregivers on therapy plans and home exercises

• Supervise therapy assistants when assigned

Required Qualifications

• Physical Therapist license (active and in good standing)

• Home Health experience required

• HCHB (Homecare Homebase) experience required

• OASIS documentation experience required

• Minimum 2 years of Physical Therapy experience preferred

• BLS or CPR certification required

• Valid driver’s license and reliable transportation required
