Carroll Hospital's hospice inpatient unit in cases of low census. Collect and document comprehensive health status data using patient records, family input, and available resources. Formulate, implement, and evaluate individualized plans of care in accordance with agency policies. Prioritize and coordinate care in a responsive and autonomous manner. Perform rounds with physicians and communicate any care changes to the hospice team. Collaborate with and supervise Hospice Aides/ HHAs

, adjusting care plans as needed and providing shift-by-shift guidance. Support performance improvement efforts by responding promptly to documentation and information requests. Serve as a resource and positive role model for team members and new staff. Actively participate in cost-effective care strategies, time management, and safe work practices. Communicate clearly and efficiently with physicians, caregivers, team members, and community partners. Maintain knowledge of hospice nursing best practices and pursue continued professional development. Represent Carroll Hospice and Carroll Hospital in the community through liaison activities.
