Full-Time Hospice Homecare/Case Manager - RN (Saratoga/Washington/Montgomery Counties) The Community Hospice has a wonderful opportunity for a full-time RN (Monday to Friday) to work as part of our Hospice Home Care Team caring for patients and families residing in Saratoga/Washington/Montgomery county area. Provide compassionate end-of-life care and support to patients and families in the comfort of their homes. What you will need: A current license to practice as a Registered Nurse in the State of New York Associates Degree in Nursing, BSN preferred 1 year experience as an RN required-preferably in Hospice/Home Care or acute care. Valid Driver's License and Reliable vehicle The RN must be able to communicate effectively, verbally and written High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers Understanding of Hospice principles preferred Training provided in end of life care. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
