Facilitator - Family Support Services

Key Functions & Responsibilities Leads and facilitates one or more of YAI’s Family Support group offerings, leading and moderating group discussions with children, adults and/or parents according to the specific program curriculum. Conducts virtual and on‑site intake sessions with participants of assigned groups to collect information regarding service needs and coordinates with leadership to ensure necessary supports are provided during group offerings. Engages with multiple participants with varying levels of verbal abilities and support needs and/or parents of children with I/DD. Works in partnership with support staff to ensure participation and engagement while maintaining a safe and structured environment. Conducts person‑centered goal setting and regularly monitors and evaluates participant progress through observations. Provides individual assistance, within a group setting, when needed. Communicates consistently with supervisor or designee, escalating programmatic concerns and reporting operational issues related to program equipment and facilities. Engages with families on‑site pre/post session regarding progress, plans and any issues or concerns that arise. Works with program leadership to identify, develop, update and enhance program curriculum according to evidence‑based practice, incorporating new teaching methods and content. Ensures timely and accurate documentation of services, attendance and billing in compliance with guidelines and regulations and in appropriate format and electronic systems. Ensures proper facility set up/clean‑up for each group session, may assist with room set up as needed. Remains current with mandatory and other training, successfully completing within required time frames. May conduct in‑person outreach presentations regarding YAI Community and Family Services (CFS) and other related programs at YAI or other locations across the boroughs of New York City. Performs all other duties as assigned. Schedule Monday – Friday; days may change based on program needs. Time: 4pm to 7pm. Minimum Qualifications Bachelor’s degree, training and/or certificate in a program‑related field or an equivalent combination of education, training and/or experience. Excellent interpersonal and communication skills. Strong problem‑solving skills with demonstrated ability to work independently. Excellent time management and written communication skills. Ability to complete room set up, including movement/sliding of tables, chairs and other equipment. Proficiency in Outlook, Microsoft Word, PowerPoint, and Excel. Preferred Qualifications One (1) year experience working with people with I/DD. Master’s degree may be required by certain contracts and/or programs. Compensation $26.15 per hour.
