Physical Therapist (PT) – Outpatient Clinic We are currently seeking a full-time Physical Therapist (PT) to join a respected outpatient clinic with a collaborative team environment and an outstanding reputation for patient care. Position Overview The Physical Therapist is responsible for evaluating and treating patients in an outpatient setting to restore function, improve mobility, reduce pain, and prevent or minimize long-term physical limitations. The PT collaborates with a multidisciplinary healthcare team to help patients of all ages recover from injuries, surgeries, and chronic conditions while achieving their rehabilitation goals. Key Responsibilities Perform comprehensive physical therapy evaluations for patients with orthopedic, neurological, musculoskeletal, and post-surgical conditions. Develop individualized treatment plans based on clinical findings, physician referrals, and patient goals. Implement evidence-based treatment interventions, including therapeutic exercise, manual therapy, gait training, balance training, and therapeutic modalities. Educate patients and caregivers on home exercise programs, injury prevention strategies, and healthy lifestyle modifications. Monitor patient progress and modify treatment plans as needed to maximize outcomes. Maintain accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries within the EMR system. Collaborate effectively with physicians and other healthcare professionals to coordinate patient care. Adhere to all facility policies, state licensure requirements, and professional standards of practice. Qualifications Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy from an accredited program. Current and active state Physical Therapy license. Previous outpatient orthopedic or hospital-based experience preferred. Strong clinical reasoning and manual therapy skills. Excellent communication, organizational, and documentation abilities.
