Responsibilities:

• Provide direct patient care, including bathing, dressing, and grooming residents according to their needs and preferences

• Assist residents with daily living activities, such as transferring, toileting, and feeding

• Support residents with mobility issues, including ambulation, transfers, and range-of-motion exercises

• Monitor and report changes in resident condition, vital signs, and admittance information to nursing staff

• Maintain accurate and detailed records of resident care and treatment

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Participate in resident activities and provide emotional support and companionship

• Maintain a clean and safe environment, adhering to infection control and safety protocols
