RN Visit Nurse

Role Overview As a Registered Nurse, you will deliver and coordinate home-based nursing care, ensuring compliance with laws, regulations, and agency policies. You will collaborate with healthcare professionals, patients, families, and community resources to develop and update individualized care plans, monitor patient progress, and ensure quality service delivery. Your responsibilities include conducting nursing assessments, verifying eligibility and medical necessity, and maintaining continuity of care from admission to discharge.
